A clinical sign of oncogenic human papilloma virus infection: urethral condyloma

Abstract

The most common clinical symptom of HPV is the condyloma acuminata, characterized by type 6 and 11 and constitutes 90% of the infections. However, 12% of the smear specimens were found to have HPV types 16 and 18, which are actually oncogenic. This virüs which frequently infects external genitalia, infects mucocutaneous component much less frequently likewise urethra as a part of mucocutaneous component. We present a case of A 58-year-old menapousal woman consulted with gynecological and obstetrics department due to pathology report of condyloma accumunata of previously excited mass from anal region by a general surgeon. At the examination of the patient, two white clitoral condyloma with white colored tiny condyloma on the right side of the urethra were seen, both HPV 16 and 18 were detected aftetwards. Urethral condyloma acuminata of atypical location at urethral mucosa which coincided with vulvar condyloma, can be a clue for oncogenic HPV infection. We aimed to discuss the mucosal involvement of HPV which is rarer but has a high probability of being oncogenic

Onkojenik human papilloma virus enfeksiyonunun klinik bir bulgusu: üretral kondilom

Abstract

Human Papilloma Virüs( HPV) enfeksiyonun en sık klinik bulgusu kondiloma akümunatadır, Genital HPV enfeksiyonlarının yaklaşık % 90'ını oluşturur ve HPV tip 6 ve 11 ile karakterizedir. fakat, bu hastaların servikal yayma preparatlarına bakıldığında % 12' sinde onkojenik tipler olan HPV tip 16 ve 18 pozitifliği söz konusudur. Sıklıkla dış genitalyayı enfekte eden bu virüs daha nadir olarak mukokütenöz yüzeyleri de enfekte edebilmektedir ve üretra da mukokütenöz yüzeyin bir parçası olması sebebiyle enfekte olabilmektedir. Genel cerrahi tarafından perianal kitle eksize edilen ve patoloji sonucu kondilom olarak gelmesi üzerine jinekolojik konsultasyon istenen 58 yaşında mennapoz döneminde olan hastayı sunmaktayız. Hastanın jinekolojik muayenesinde 2 adet beyaz klitoral ve üretranın sağ sınırını oluşturan1 adet minik beyaz kondilom izlendi ve muayene sırasında alınan sürüntü örneğinde ise HPV 16 ve 18 pozitifliği saptandı. Üretral mukoza gibi atipik yerleşimli ve eş zamanlı vulvar kondilomu olan üretral kondiloma akümünatanın tespit edilmesi onkojenik HPV enfeksiyonlarının mukozal tropizmi sebebi ile, onkojenik surşların enfeksiyonunun araştırılması için bir ipucu olabilir. Vakamız eşliğinde HPV’nin daha nadir fakat onkojenik olma ihtiali yüksek mukozal tutulu tartışmayı amaçladık.
