Abstract
The reasons of the high rate failure of osteoarthritis and osteonecrosis with total hip prosthesis on the young, active andbelow-55-year-old patients on the long run are osteolizis due to polyethylene corrosion accelerated by high activity andperiprostatic bone loss. Because of this, by adjusting corrosion-resistant surfaces into total prosthesis, surface changinghip arthroplasty which has a great advantage protecting proximal femoral bone stocks has become a current issue. Inthis case report, we define a 45-year-old patient who has bilateral grade 2-3 hip avascular necrosisis, had haddecompression and bone marrow derivation but still has complaints, and who had bilateral total hip surface changingarthroplasty because of progressive necrosis.

Abstract
Osteoartrit ve osteonekrozun, 55 yaş altı genç ve aktif hastalarda uzun dönemde total kalça protezi ile yüksek oranda başarısızlığın nedenleri, yüksek aktivitenin hızlandırdığı polietilen aşınmasına bağlı osteolizis ve periprostetik kemik kaybıdır. Bunun için aşınmaya dayanıklı yüzeylerin total protezlere uyarlanmasıyla, proksimal femoral kemik stoğunu koruyarak büyük bir avantaj sağlayan, yüzey değiştirme kalça artroplastisi gündeme gelmiştir. Biz olgumuzda 45 yaşında, bilateral grade 2-3 kalça avasküler nekrozu (AVN) olan, daha önce dekompresyon ve kemik iliği derivasyonu uygulanan, fakat hastanın şikayetlerinin geçmemesi ve nekrozun ilerlemesi üzerine bilateral total kalça yüzey değiştirme artroplastisi yapılan bir hastayı tanımladık.
