Abstract
Xylazine
ve Ketamin veteriner hekimlikte hayvanları bayıltmak amacıyla sıkça birlikte
kullanılan anestezik ilaçlardır. Ketaminin insanda kullanımı olsada Xylazine
tek başına ciddi hipotansiyon yapıcı etkisinden dolayı insanlarda
kullanılmamaktadır. Vakamız 29 yaşında kamu personeli idi ve kazayla vücuduna
200 mg ketamin ve 100 mg xylazine karışımı bulunan iğne saplanmıştı. Hastamızın
acil servis gelişinde uykuya meyil ve hafif taşikardisi dışında ek şikayeti
olmasa da takibinde ciddi hipotansiyon ve,bradikardi atağı gelişti. 24 saatlik
takip sonrası hasta taburcu edildi. Literatürde kazayla gelişen xylazin ve
ketamin karışım vakaları nadir rastlanmaktadır. Acil servislere rutin
insanlarda kullanılan ilaçlarla olan toksikolojik vakalar dışında farklı
canlılarda kullanılan ve klinik pratikte nadir görülebilen toksikoloji vakaları
da başvurmaktadır. Bu tür vakaların klinik stabil seyretse de ani gelişebilecek
anstabilteye bağlı mortaliteye de yol açabileceği unutulmamalıdır.
