CHILAIDITI'S SYNDROME; IT SHOULD NOT BE REASONED OF UNNECESSARY SURGERY IN THE PATIENTS WITH TRAUMA

Abstract

Chilaiditi’s syndrome is rare and usually incidentally diagnosed and the interposed colon and small intestine are located between liver and right diaphragm. A 25-year-old man had admitted to another hospital after blunt trauma caused by explosion. He transferred to our hospital with the diagnosis of traumatic diaphragmatic rupture for surgical treatment. As a result of clinical and radyological evaluation Chilaiditi’s syndrome was diagnosed. Surgical treatment is not occasionally required in cases without complication. Clinicians should keep in mind Chilaiditi’s syndrome in order to avoid unnecessary surgery in the differential diagnosis of patients with abnormality of the diaphragm after blunt trauma.

Abstract

Chilaiditi sendromu kolon ve ince barsakların interpozisyonu sonucu karaciğerle sağ hemidiyafragma arasında yerleştiği, nadir görülen ve genellikle tesadüfen saptanan bir anamolidir. 25 yaşında erkek hasta araç içerisinde patlayıcıya bağlı künt travma nedeniyle başka bir merkeze başvurmuş. Diyafragma rüptürü tanısı ile cerrahi tedavi amacıyla hastanemize gönderildi. Hastanın kliniği ve radyolojik tetkikleri birlikte değerlendirildiğinde Chilaiditi sendromu tanısı konuldu. Komplikasyon gelişmediği sürece genellikle ameliyat gerektirmeyen bu anomali tanısal yanılgılara neden olabilmektedir. Künt travma sonrası diyafragma anormalliği izlenen hastalarda, gereksiz cerrahiden kaçınmak için Chilaiditi sendromu ayırıcı tanıda aklıda bulundurulmalıdır.
