Aşırı aktif Mesane'de İdrar Kaçırmanın Kadın Cinsel Fonksiyonuna Etkisi

Abstract

Giriş:
Aşırı aktif mesane(AAM) olan olguların %36sında sıkışma inkontinansı görülmektedir ve bu en can sıkıcı bulgudur. Cinsel disfonksiyon birçok kadının yaşam kalitesini olumsuz etkilemekte ve erkeklerdeki cinsel disfonksiyondan daha sık görülmektedir. Bu çalışmada AAM si olan kadınların cinsel fonksiyonlarını değerlendirmeyi amaçladık.
Materyal ve Metot:
Eylül 2019- Şubat 2020 tarihleri arasında aşırı aktif mesane şikayetiyle başvuran cinsel aktif kadın hastalar çalışmaya alınmak için değerlendirildi. İdrar kaçırması olan 32 hasta ve kaçırması olmayan 32 hastadan toplam 64 hasta çalışmaya dahil edildi. Tüm hastalara OAB-V8 ve Kadın Cinsel işlev Ölçüm (KCİÖ) sorgulama formları doldurtuldu. Kontinan ve inkontinan hastalar cinsel fonksiyon ve KCİÖ alt gruplar açısından karşılaştırıldı.
Bulgular:
64 hastanın 31 (%48.4)’inde cinsel disfonksiyon mevcuttu. İnkontinan hastaların 24(%75)’ünde cinsel disfonksiyon varken, kontinan olanların 7(%21.9)’sinde cinsel disfonksiyon vardır (p<0.01) idrar kaçırması olan 32 hastanın 6 (%18.8)’sı idrar kaçırmaya bağlı orgazm olamadığını söylerken, 28 (%87.5)’i idrar kaçırma korkusundan dolayı orgazm olamadıklarını belirtti. Diğer bir ifadeyle, idrar kaçırma korkusundan dolayı orgazm olamayanların 22 (%78.6)’sinde cinsel disfonksiyon vardı.
Sonuç:
Bu çalışmada AAM'li kadınlarda idrar kaçırmanın cinsel işlev bozukluğu ile ilişkisi ve cinsel aktivitenin temel amaçlarından biri olan orgazma ulaşamamayla idrar kaçırma endişesinin ilişkilendirilebileceği gösterilmiştir. AAM tedavi edildiğinde cinsel işlev bozukluğunun düzeldiğini gösteren çalışmalara ihtiyaç vardır.

The Effect of Urinary Incontinence on Female Sexual Dysfunction in Overactive Bladder

Abstract

Abstract
İntroduction
Urge incontinence is observed in 36% of cases with overactive bladder (OAB) and this is the most bothersome finding. Sexual dysfunction negatively affects the quality of life of many women and is more common than in men. In this study, we aimed to evaluate the sexual functions of women with OAB.
Material and Methods
Patients who presented with overactive bladder complaints between September 2019 and February 2020 were evaluated. A total of 64 patients from 32 patients with urinary incontinence and 32 patients without urinary incontinence were included in the study. Over Active Bladder -V8 (OAB-V8) and Female Sexual Function Index (FSFI) questionnaire forms were filled in all patients. Patients with continence and incontinence were compared in terms of sexual function and FSFI subgroups.
Results
Of the 64 patients, 31 (48.4%) had sexual dysfunction. While 24 (75%) of patients with incontinence had sexual dysfunction, 7 (21.9%) of the continent ones had sexual dysfunction (p <0.01). While 6 (18.8%) of 32 patients with urinary incontinence said that they could not have orgasm due to urinary incontinence, 28 (87.5%) stated that they could not have orgasm due to fear of urinary incontinence. In other words, 22 (78.6%) of those who could not orgasm due to fear of urinary incontinence have sexual dysfunction.
Conclusion
In this study, urinary incontinence in women with OAB has been shown to be associated with sexual dysfunction. It has also been shown that anxiety of urinary incontinence can be associated with an inability to reach orgasm, one of the main goals of sexual activity. There is a need for studies showing that sexual dysfunction improves when OAB is treated.
