A Turkish Validity and Reliability Study of The Decisional Balance Scale in Obese and Highly Weight People

Abstract

Background: Obesity is an important health problem that develops due to environmental, genetic, neurological and psychosocial factors. Obesity management should be comprehensive, continuous and feasible. In terms of creating behavioral changes in obesity, the Decisional Balance Scale will benefit health professionals working in primary care, since it provides information about the stage of the patient and clinical approaches can be arranged more effectively accordingly. Method: In our study, it was aimed to introduce the transtheoritical model, the decisional balance scale in obese and overweight individuals, to the literature by conducting a Turkish validity and reliability study. Results: The 14-item scale was grouped under two factors, and the so-called title covered by 47.5% of all statements. It is not similar to the original scale due to the separation of these two positive and negative statements. "Smoking Cessation Decision Making Balance Scale" was used for criterion validity. Test-retest, split half and cronbach alpha analyzes were performed for reliability. The correlation coefficient obtained from the test and retest results was = 0.978 and p <0.05, and it was found to be significant as used. Guttman Calculated Split Half = 0.812 and p <0.05 used as used. Cronbach alpha coefficient = 0.762 Spearman Brown = 0.821, was found as an internal discovery indicator for reliability. The Cronbach alpha coefficients of the positive and negative scores of the decisional balance scale were calculated separately again, the positive decisional balance Cronbach alpha coefficient was 0.812 and the negative decisional balance Cronbach alpha coefficient was 0.721. As a result of the analyzes, it has been shown that the 14-item form of the Weight Loss Desicional Balance Scale, developed by O'Connel and Velicer (1988), is valid and reliable in Turkish. Conclusion: In primary care, which is the first point of contact with the patient; It is important to raise awareness for a long-term healthy life, to follow up closely, to suggest lifestyle changes, and to reduce the risk with pharmacological treatment when necessary, with recommendations such as exercise, diet, and smoking cessation. By using the decisional balance scale, it will help family physicians and health professionals working in primary health care services to develop approaches and take precautions for patients by revealing their thoughts about their readiness for behavior change.

Obez ve Fazla Kilolu Kişilerde Karar Dengesi Ölçeğinin Türkçe Geçerlik ve Güvenirlik Çalışması

Abstract

Giriş:Obezite; çevresel, genetik, nörolojik ve psikososyal etkenlere bağlı olarak gelişen önemli bir sağlık problemidir. Obezite yönetimi kapsamlı, sürekli ve uygulanabilir olmalıdır. Obezitede davranış değişikliği yaratma açısından Karar Dengesi Ölçeği hastanın hangi aşamada olduğu konusunda bilgi vermesi ve buna göre klinik yaklaşımların daha etkili düzenlenebileceğinden dolayı birinci basamakta çalışan sağlık profosyonellerine yarar sağlayacaktır. Yöntem: Çalışmamızda, obez ve fazla kilolu kişilerde Karar Dengesi Ölçeğinin Türkçe geçerlilik ve güvenilirlik çalışması yapılarak literatüre kazandırılması hedeflenmiştir. Bulgular: 14 maddelik ölçeğin iki faktör altında toplanarak, tüm ifadelerin %47,5’inden fazlasını kapsadığı gösterilmiştir. Bu iki faktör olumlu ve olumsuz ifadeleri ayırması nedeni ile ölçeğin orijinaline benzemektedir. Ölçüt geçerliği için “Sigara Bırakma Karar Verme Dengesi Ölçeği”nden yararlanılmıştır. Güvenirlik için test tekrar-test, splithalf ve cronbach alfa analizleri yapıldı. Test ve tekrar-test sonucu elde edilen korelasyon katsayısı=0,978 ve p <0,05 olup istatistiksel olarak anlamlı bulunmuştur. GuttmanSplitHalf=0,812 olarak hesaplanmış ve p<0,05 olup istatistiksel olarak anlamlı bulunmuştur. Güvenirlik için iç tutarlılık göstergesi olarak Cronbach alfa katsayısı=0.762 Spearman Brown=0.821, bulunmuştur. Karar Dengesi Ölçeği pozitif ve negatif puanlamalarının cronbach alfa katsayıları tekrar ayrı ayrı hesaplanmış, pozitif karar dengesi Cronbach alfa katsayısı 0,812 ve negatif karar dengesi Cronbach alfa katsayısı 0,721 bulunmuştur. Yapılan analizler sonucu Weight Loss Desicional Balance Scale “Zayıflama Karar Verme Dengesi Ölçeği” nin, 14 maddelik formunun Türkçe geçerlilik ve güvenilirliğinin olduğu gösterilmiştir. Sonuç: Hasta ile ilk temas noktası olan birinci basamakta; obezite açısından risk grubundaki bir hastaya egzersiz, diyet, sigarayı bırakmak gibi önerilerle uzun dönem sağlıklı yaşam için bilinçlendirmek, yakın takip altına almak, yaşam tarzı değişiklikleri önermek ve gerektiğinde farmakolojik tedavi ile riski azaltmak önemlidir. Karar Dengesi Ölçeği kullanılarak hastaların davranış değiştirme konusundaki hazırlık durumları ve düşünceleri ortaya çıkarılarak hastalara yönelik yaklaşımlar geliştirilmesi ve önlemler alınması konusunda aile hekimlerine ve birinci basamak sağlık hizmetlerinde çalışan sağlık profesyonellerine katkı sağlayacağı düşünülmektedir.
