KURUM İÇİ VE KURUMLAR ARASI KRİTİK HASTA TRANSFERİNDE HEMŞİRELİK BAKIMI

Abstract

Genellikle yoğun bakım
ünitelerinde takip edilen kritik hastalar; görüntüleme birimi,  acil servis, ameliyathane gibi alanlar arası
nakil ya da özel ekipman/merkez gereksinimleri nedenleriyle kurum içi ve
kurumlar arası sıklıkla transfer edilmektedirler. Fakat kritik hasta transferi,
taşıma sürecinde istenmeyen olay ve komplikasyonların yaşanması, yoğun bakım
ünitesinde kalış süresi ve mortalite oranlarının artışı gibi sonuçlara yol
açtığından yüksek risk taşımaktadır. Kritik hasta transferinin güvenli bir
şekilde gerçekleştirilebilmesi ve istenmeyen olay ve komplikasyonların
önlenebilmesi için, hastaların transfere hazırlanması, koordinasyon ve
iletişim, eşlik edecek sağlık personelleri, gerekli ekipman, transfer süresince
izlem ile kayıt ve raporlama unsurlarına uyulması ve özen gösterilmesi
gerekmektedir. Bu derlemenin amacı, kurum içi ve kurumlar arası kritik hasta
transferinde benzer olan bu unsurların incelenmesi ve hemşirelik bakım
uygulamalarının tartışılmasıdır.

Nursing Care in Intra- and Inter-Hospital Transfer of Critically Ill Patients

Abstract

Critically ill patients, who are usually followed up
in an intensive care unit, are often transferred intra- and inter-hospital due
to transport between areas such as imaging unit, emergency room, operating
room, or special equipment/center requirements. However; since adverse events
and complications experienced during the transport lead to an increase in
mortality and length of stay in the intensive care unit, transfer of the
critically ill patient carries high risks. It is necessary that attention is
paid and follows to the preparation of patients for transfer, coordination and
communication, health personnel accompanying the patient, equipment, monitoring
during transfer, and
documentation
and reporting components
in order to ensure the safe transfer of critically
ill patients and to prevent adverse events/ complications. This review aims to
examine these components common to intra- and inter-hospital transfer of
critically ill patients and to discuss nursing care practices.
